Q. What are my insurance benefits?
A. We will call your insurance company after you have scheduled an appointment in our office, and will be happy to provide you with an estimate of your benefits after you have seen the doctor and a diagnosis has been provided. If you would like to know your insurance benefits prior to scheduling an appointment in our office, you will need to contact your insurance company directly.

Q. How much is this going to cost?
A. We will be happy to give you an estimate over the phone, but the exact amount cannot be given until the doctor sees you at the time of your consultation and makes a diagnosis.

Q. Will this be covered by my medical insurance or dental insurance?
A. Procedures dealing with teeth and gums will most likely be covered by your dental insurance. All other procedures will most likely be covered by your medical insurance. If the proper insurance information has been provided to our office prior to your initial visit, we will be able to give you an estimate of insurance benefits at the time of your initial visit.

Q. Do I need to fill out paper work?
A. You will need to fill out patient information and a health history before being seen by one of our doctors. You will also be asked to sign a financial agreement.

Q. Does my dentist know your fees?
A. Our referring dentists are not aware of our fees. You will have to contact our office directly to discuss our fees.

Q. Is there a charge for my post-op appointment?
A. There is no charge for your initial post-op appointment.

Q. Can I schedule my initial visit on a Friday?
A. We reserve our Friday appointments for patients who have already been seen by one of our doctors for their initial consultation and are now returning for surgery.

Q. Do you have a payment plan?
A. Payment is due at the time of service. We accept personal checks, cash, Visa, MasterCard, and Discover. Personal checks will be verified the day they are presented for payment.

Q. What do I need to bring to my first visit at your office?
A. Please bring your referral and any appropriate x-rays. You may have your referring doctor send your x-ray, or you can pick it up and bring it with you at the time of your appointment.

Q. Do I need to arrive early for my appointment?
A. If this is your first visit to our office, we ask that you arrive 15 minutes prior to your appointment to fill out the necessary paper work. If you are returning for surgery, please arrive 5 minutes before your scheduled appointment time.

Q. Can I reach a doctor on the weekends?
A. One of our doctors will be available at all times. Please call (602) 493-1140 if you need to reach one of our doctors after hours, and our answering service will assist you in contacting the doctor that is on call.
