He received a B.S. degree from Tulane University in New Orleans. He attended dental school at Washington University in St. Louis and received his D.M.D. degree. Dr. Kootman then completed his residency in Oral and Maxillofacial Surgery at Michael Reese Hospital in Chicago.
Dr. Kootman has been president of the Arizona Society of Oral Surgeons and is a member of many societies.
Dr. Kootman has two wonderful children Adam and Amy. He enjoys competitive cycling, running, and swimming.
